Here’s how to create your password:
If it is your first time logging in or you have forgotten your log-in information, please use the “Forgot Password” function to set your password. Please save this password for reference, as you will access the Patient Portal throughout your Aligned Modern Health journey. Once you set your password, your username will be your email address.

If you have any issues completing the intake forms before your scheduled appointment time, please alert us with urgency. We are here for you and happy to help! *We recommend using a computer or tablet to complete the forms, not a cellular phone.

If you need to reschedule or cancel your appointment, be sure to do so at least 24 hours in advance of your appointment time to avoid a $50 fee (or 50% of the scheduled service amount for IV Vitamin Therapy).
