Uploading Your Insurance Card

If you are utilizing an in-network insurance policy at Aligned Modern Health, a copy of your current insurance card must be on file along with a copy of your photo ID.

Please upload a copy of your photo ID and the *front and back* of your insurance card into your <<Patient Portal>> using the following instructions:

  1. Save scans/images of your photo ID and the *front and back* of your insurance card to your device 
  2. Click the three (3) horizontal lines on the top left side of your portal screen 
  3. Click on “Documents”  
  4. Click on the “External Documents” folder  
  5. Click on “Browse Files” to upload the images previously saved in step one  
  6. Double check that all three (3) images are now in the folder:  photo ID, front of insurance card, back of insurance card

Need to Reschedule? Cancellation / No-Show Policy

If you need to reschedule or cancel your appointment, be sure to do so at least 24 hours in advance of your appointment time to avoid a $50 fee (or 50% of the scheduled service amount for IV Vitamin Therapy).
