If you have not already submitted your preferred credit card, HSA card, or FSA card information, please call (773) 692-6700 to complete this required step

Self-pay rates are charged at the time of each visit. 

Patients utilizing insurance for their visits will be charged the minimum benefit amount at the time of each visit. Any remaining balance after the insurance claim processes will first be communicated on a patient statement via email and then auto-charged within three (3) days of the statement date.   (PDF of Collections Policy)

Friendly Reminder: Cancellation / No-Show Policy

You are welcome to reschedule your visit more than 24 hours before your appointment time.  If a scheduled appointment is missed or canceled within 24 hours of the scheduled appointment time, a $50 cancellation fee will be automatically charged to the card on file.

If you need to reschedule or cancel your appointment, be sure to do so at least 24 hours in advance of your appointment time to avoid a $50 fee (or 50% of the scheduled service amount for IV Vitamin Therapy).
