Welcome to your Functional Medicine journey with Aligned Modern Health! 

 

New Patient Functional Medicine visits via telemedicine are approximately 60 minutes in length and include a full health history and review of concerns and goals.  You will receive an emailed link to join your telemedicine appointment at the start time of your visit (or potentially a few minutes after, given the rely on technology).  Please refresh your inbox until you receive the link.   

Please be prepared to candidly discuss all of your concerns and goals with your provider.   

 

If you are able to, please upload any of the following to your Patient Portal prior to your visit:  

  • Lab or test results from other medical providers (within the last year) 
  • Medical records from other medical providers (within the last year) 
  • Current prescription medication(s) and supplements (photos in the original bottles)   

 

To do so: 

  1. Save scans/images of test results, medical records, and/or medication photos to your device 
  2. Click the three (3) horizontal lines on the top left side of your portal screen 
  3. Click on “Documents”  
  4. Click on the “External Documents” folder  
  5. Click on “Browse Files” to upload the images previously saved in step one  
  6. Double check that all items are saved in the folder 

 

In most cases, your provider will prescribe one or more tests to help guide your custom treatment plan.  These tests may include blood work or the collection of other bodily samples (saliva, urine, and/or stool).  Your provider will review the purpose and goals of these recommendations with you at your visit, along with associated costs.  While some tests may be billed through insurance, others are fully self-pay.   

For your convenience, Aligned Modern Health offers phlebotomy (blood work, urine) appointments right from the comfort of many of our clinic locations.  Out-of-state patients may schedule at a phlebotomy site convenient to the home or office.  Additional test kits (saliva, stool) are administered/collected in the home (and may be shipped to out-of-state patients).  Your provider and/or clinic manager will help with scheduling phlebotomy appointments and/or purchasing test kits, if applicable.

Friendly Reminders

 

Please visit your secure Patient Portal and complete the required intake forms at least 24 hours prior to your visit. 

Please allow 40-45 minutes to complete these forms.   

If it is your first time logging in or you have forgotten your log-in information, please use the “Forgot Password” function to set your password. Please save this password for reference, as you will access the Patient Portal throughout your Aligned Modern Health journey. Once you set your password, your username will be your email address 

If you have any issues completing the intake forms prior to your scheduled appointment time, please alert us with urgency. We are here for you and happy to help!  *We recommend using a computer or tablet to complete the forms, not a cellular phone  

 

Insurance Information:

If you are interested in utilizing an in-network insurance policy for your visit and you have not already submitted your policy information, please click here to review in-network providers and request a verification of benefits.  You are required to bring your insurance card with you to your visit or you may upload the front and back of the insurance card to the Patient Portal

 

Credit Card Information and Collections Policy:  

If you have not already submitted your preferred credit card, HSA card, or FSA card information, please call (773) 692-6700 to complete this required step.  Self-pay rates are charged at the time of each visit.  Patients utilizing insurance for their visits will be charged the minimum benefit amount at the time of each visit. Any remaining balance after the insurance claim processes will first be communicated on a patient statement via email, and then auto-charged within three (3) days of the statement date.   (PDF of Collections Policy) 

 

Cancellation / No Show Policy: 

You are welcome to reschedule your visit more than 24 hours in advance of your appointment time.  If a scheduled appointment is missed or cancelled within 24 hours of the scheduled appointment time, a $50 cancellation fee will be automatically charged to the card on file.
