We accept all major health insurance at Aligned Modern Health and are in network with many plans. If we are not part of your network, we can provide you with the documentation that you need to submit to your insurance company for direct reimbursement. In addition, Flexible Spending Accounts and Health Care Reimbursement accounts may be used for many of our services as well. If you have questions about your insurance coverage, our team will verify your coverage and explain your benefits before treatment, at no charge.

Flexible Spending Accounts and Health Savings Accounts

Our services are eligible expenses for most flexible spending accounts (FSA), health savings accounts (HSA), and health reimbursement accounts (HRA).

Self Pay Packages

We offer many accessible options for self-payment including monthly memberships and packages for many of our services. To obtain our time of service self-pay rates, please request more information by filling out the form below.

Personal Injury/Worker's Compensation

If you’ve been injured in an accident or while at work, your treatment may be covered. Please contact us directly and we can tell you about reimbursement and the required paperwork needed.

Financial Policy – Insurance-based Visits

At the time of your visit, we’ll collect the minimum amount due based on your insurance coverage. Once your insurance processes your claim, we’ll send you a Statement of Charges and charge your credit card for the remaining balance. For many patients, there will likely be a balance owed once insurance processes due to specific insurance coverage terms and deductible rates.
