Friendly Reminders
Intake Forms:
Please visit your secure << Patient Portal >> and complete the required intake forms at least 24 hours prior to your visit.
Please allow 5-10 minutes to complete these forms.
If it is your first time logging in or you have forgotten your log-in information, please use the “Forgot Password” function to set your password. Please save this password for reference, as you will access the Patient Portal throughout your Aligned Modern Health journey. Once you set your password, your username will be your email address.
If you have any issues completing the intake forms prior to your scheduled appointment time, please alert us with urgency. We are here for you and happy to help! *We recommend using a computer or tablet to complete the forms, not a cellular phone.
Insurance Information:
If you are interested in utilizing an in-network insurance policy for future visits and you have not already submitted your policy information, please click here to review in-network providers and request a verification of benefits.
Cancellation / No Show Policy:
You are welcome to reschedule your visit more than 24 hours in advance of your appointment time. If a scheduled appointment is missed or cancelled within 24 hours of the scheduled appointment time, a $50 cancellation fee will be automatically charged to the card on file.
