Welcome to your Clinical Massage Therapy journey with Aligned Modern Health! 

 

Standard New Patient Massage visits are approximately 60 minutes in length, with approximately 50-minutes of hands-on treatment.  Your appointment includes time for consultation with your provider and changing.   

At your visit, you may undress to the level of your comfort.  Please be prepared to allow access to all areas of concern, though not all areas require skin-to-skin contact with your provider.   

You are encouraged to communicate with your provider at any time during your visit, especially regarding the amount of pressure and/or style of treatment being administered.  Some clinically indicated massage therapies may produce intense feelings in muscles, soft tissues, and skin, but you should never be in pain for an elongated period of time.   

After your treatment, you are strongly advised to drink plenty of fresh water (non-carbonated).  Your provider may also suggest the application of heat or ice later in the day.

Please visit your secure Patient Portal and complete the required intake forms at least 24 hours prior to your visit. 

Please allow 20-25 minutes to complete these forms.   

If it is your first time logging in or you have forgotten your log-in information, please use the “Forgot Password” function to set your password. Please save this password for reference, as you will access the Patient Portal throughout your Aligned Modern Health journey. Once you set your password, your username will be your email address 

If you have any issues completing the intake forms prior to your scheduled appointment time, please alert us with urgency. We are here for you and happy to help!  *We recommend using a computer or tablet to complete the forms, not a cellular phone  

 

Insurance Information:

Clinical Massage Therapy on its own is not covered by insurance. If you are interested in utilizing an in-network insurance policy for your visit because of an acute medical concern, you may consider a Chiropractic Physical Medicine treatment plan. If manual therapy is clinically indicated and prescribed by the physician as a part of the treatment plan, it may qualify for insurance coverage. Please contact our team at (773) 692-6700 if you have any questions.  

 

Credit Card Information and Collections Policy:  

If you have not already submitted your preferred credit card, HSA card, or FSA card information, please call (773) 692-6700 to complete this required step.  Self-pay rates are charged at the time of each visit. 

 

Cancellation / No Show Policy: 

You are welcome to reschedule your visit more than 24 hours in advance of your appointment time.  If a scheduled appointment is missed or cancelled within 24 hours of the scheduled appointment time, a $50 cancellation fee will be automatically charged to the card on file.
