Welcome to your IV Vitamin Therapy journey with Aligned Modern Health! 

 

New Patient IV visits are approximately 60 minutes in length. Each infusion is prepared daily to order and contains customized, high-quality ingredients made in the USA and regulated by a US pharmacy. IVs are a safe and effective way to deliver nutrients and fluids directly into the bloodstream.  

Side effects are often mild or non-existent, and may include pain at the injection site, swelling, and/or bruising. 

Please make sure that you eat something within two (2) hours prior to your appointment.  

Please wear a short-sleeved shirt.  You may request which arm is accessed to administer the treatment, but your provider will ultimately select based on the best possible vein option.   

You are welcome to bring a book, magazine, or electronic device to enjoy during your treatment. If you choose to use sound on an electronic device, please bring headphones or ear buds so as to not disturb other patients.

Friendly Reminders

 

Please visit your secure Patient Portal and complete the required intake forms at least 24 hours prior to your visit. 

Please allow 10-15 minutes to complete these forms.   

If it is your first time logging in or you have forgotten your log-in information, please use the “Forgot Password” function to set your password. Please save this password for reference, as you will access the Patient Portal throughout your Aligned Modern Health journey. Once you set your password, your username will be your email address 

If you have any issues completing the intake forms prior to your scheduled appointment time, please alert us with urgency. We are here for you and happy to help!  *We recommend using a computer or tablet to complete the forms, not a cellular phone  

 

Insurance Information:

IV Vitamin Therapy is not covered by insuranceThat said, you may use an HSA or FSA account to pay for your services

 

Credit Card Information and Collections Policy:  

If you have not already submitted your preferred credit card, HSA card, or FSA card information, please call (773) 692-6700 to complete this required step.  Self-pay rates are charged at the time of each visit. 

 

Cancellation / No Show Policy: 

You are welcome to reschedule your visit more than 24 hours in advance of your appointment time.  If a scheduled appointment is missed or cancelled within 24 hours of the scheduled appointment time, a $50 cancellation fee will be automatically charged to the card on file.
