Understanding insurance can feel overwhelming. At Aligned Modern Health, we work with many major insurance plans and help you understand what to expect before, during, and after your care.

Insurance and Self-Pay Options

Insurance

  • When we are in-network with your insurance provider, we submit claims on your behalf
  • Coverage varies by plan, employer, and individual benefits
  • Final coverage is determined by your insurance provider after claims are processed

Self-Pay & Payment Options

  • Self-pay options are available to patients not using insurance
  • Most services are eligible for FSA and HSA funds

How Insurance Works at Aligned Modern Health

We guide you through each step so you know what to expect before, during, and after your visit

Before Your Visit

  • Copays: Collected prior to your appointment when required by your plan
  • Coverage: Confirmed directly with your insurance provider
  • Deductibles: If you haven’t met your deductible, you’ll likely pay the full contracted rate for services until it’s met. We will bill you after your insurance processes.

During Your Care

  • We will gladly submit your covered services to your insurance on your behalf
  • Some specialty testing, services and prescriptions will not covered by insurance
  • Your doctor or clinical care coordinator will review these details with you before ordering

After Insurance Processes

  • Once your insurance has processed your claim, and we understand the patient responsibility balance you owe, you’ll receive an email and text along with a detailed statement in your patient portal
  • The statement will show what your insurance covered and what you owe
  • Your card on file will be charged the same day or within a few days and a receipt will be emailed

Understanding Common Insurance Terms

Copays

A fixed amount you pay for a visit or service, typically due at the time of care.

Deductibles

The amount you pay out of pocket before your insurance begins covering eligible services.

Coinsurance

Your share of costs for covered services after your deductible is met, usually expressed as a percentage.

Self-Pay

Self-pay simply means you choose to pay out of pocket for your care, without involving an insurance provider.

In-Network by Service

In-network status varies by plan, service, and location. This table reflects common coverage patterns but does not guarantee coverage. Final determination is made by your insurance provider.

Labs, Testing, and Specialty Services

Some services fall outside standard office visits and are billed separately.

Labs

Some or all of your lab costs may be part of your insurance benefits. Please note depending what lab tests are ordered, you may receive billing from both Aligned Modern Health and our lab partners.

Self-Pay & Specialty Test Kits

Please note specialty test kits are not reimbursable by insurance.

Medications & Compounded Prescriptions

Your personalized compound prescription and nutraceuticals will not be covered by insurance and will be reviewed with you before ordering.

Service-Specific Coverage

Coverage varies by service and plan. Our team can help clarify and answer your questions before care begins, however coverage must be confirmed by your insurance provider.
