Treat this card as cash. It cannot be replaced if lost or stolen. This card is redeemable at all Aligned Modern Health locations.

Redemption:
Gift Cards are redeemable towards in-clinic services and/or telemedicine services, whether billed to insurance or self-pay.
Gift Cards may also be redeemed towards IV Vitamin Therapy treatments, labs/test kits, and supplements/retail items purchased in-clinic.
Gift Cards may not be used for recurring charges/repeat autopay occurrences (including wellness plans and memberships that are on an automatic recurring payment).
Bonus Card Terms
Applicable only during specific promotional periods: for every $100 in gift cards purchased, receive a bonus $25 card!
Bonus Cards may be redeemed towards single-visit, self-pay clinic services ONLY.
Bonus Cards may not be used towards insurance-based visits (co-pays, co-insurance, deductibles, etc., or for any portion of a visit billed to Medicare, Medicaid, or Tricare), tests, lab kits, supplements, IV Vitamin Therapy, or retail product purchases (in-clinic or online).
Bonus Cards may not be used for recurring charges/repeat autopay occurrences (including wellness plans and memberships that are on an automatic recurring payment).
Limit one Bonus Card per visit/transaction. Bonus Cards expire 90 days from date of issue.
