Bonus cards may be redeemed towards self-pay clinic services ONLY. Bonus cards may not be used towards insurance-based visits (co-pays, co-insurance, deductibles, etc., or for any portion of a visit billed to Medicare, Medicaid, or Tricare), tests, lab kits, supplements, IV Vitamin Therapy, or retail product purchases (in-clinic or online). Limit one bonus card per visit/transaction. Bonus cards expire 90 days from date of issue.
